Job Summary:
We are seeking a skilled and compassionate Primary Care Physician to join our medical team. Your expertise and dedication will contribute to the overall well-being of our patients.

Key responsibilities include:

Essential Functions

Provide primary medical care by caring for all patients in accordance with the physician’s medical specialty; develops a plan of care for each patient, including complete medical history, physician examination, diagnosis, appropriate treatment and/or referral, including hospitalization, when necessary; stresses the importance of and keeps in compliance with clinical guidelines relating to preventive health care measures; and uses all available resources in diagnosis and treatment, such as laboratory and radiological testing, in an appropriate and cost-efficient manner.

Confer with consulting physicians, nurses, patients, and patient’s families concerning treatment and care of patients; refers those cases, which require specialist services, but maintains responsibility, assuring that continuity of care is provided; serves as a clinical preceptor relating to medical education; educates support staff where needed to maintain patient safety and workflow; completes electronic health records according to practice standards; and closes charts within the appropriate time frame.

Participate in the development and implementation of a regularly up-dated performance improvement plan for the consortium and disease collaborative(s); participates in the Health Promotion/Disease Prevention activities required by the U.S. Public Health Services; and participates in the health center’s quality assurance program, including attending morning huddles to help maintain Patient Centered Medical Home recognition and Joint Commission Accreditation.

Perform administrative support work such as word processing, creating spreadsheets, data entry and retrieval; orders, monitors and distributes program supplies/resources; and other information to ensure accuracy and conformance to established procedures and policies; and counsels patients regarding patient responsibilities and rights; adheres to patient confidentiality guidelines. Perform other duties as assigned.

Required Qualifications:

Graduation from an accredited medical school with a degree of Doctor of Medicine or Osteopathy.  Completion of an approved residency program in their field.  License to practice medicine in the Commonwealth of Virginia. Board certified in their field.  Must be certified in Basic Life Support techniques.

Requires an acceptable background check to include state and local criminal check; satisfactory results from a pre-employment medical evaluation and substance abuse testing and is subject to random alcohol and controlled substance testing.  As a condition of continued employment, you must complete a state-mandated application for credentialing and recertification.

Required Knowledge

  • Patient Care– Provide health care services to patients, families, and communities aimed at preventing health problems or maintaining health. Perform all medical, diagnostic, and surgical procedures considered essential for the area of practice. Gather essential and accurate information about patients and their conditions through history-taking, physical examination, and the use of laboratory data, imaging, and other tests. Organize and prioritize responsibilities to provide care that is safe, effective, and efficient. Interpret laboratory data, imaging studies, and other tests required for the area of practice. Make informed decisions about diagnostic and therapeutic interventions based on patient information and preferences, up-to-date scientific evidence, and clinical judgment. Develop and carry out patient management plans aimed at preventing, maintaining, or improving health problems and/or outcomes. 
  • HIPAA– Knowledge of the federal Health Insurance Portability and Accountability Act (HIPAA) to ensure privacy and safeguard patient information. 
  • Customer Service— Considerable knowledge of principles and processes for providing customer service. This includes meeting quality standards for service.
  • Technology— General knowledge of providing and documenting healthcare using electronic health records (EHR).

Education: Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
Licensure: Current unencumbered licensure as an Advanced Practice
Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialty
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice and/or Palliative background preferred.
Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs.
• Conduct comprehensive assessments
• Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
• Manage members during a short term sub-acute rehabilitation episode
• Ensure accurate and complete documentation, including ICD 10 conditions
• Communicate and collaborate with the interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide patients and caregivers with counseling and educationMust love building relationships with patients, value bedside manner, and possess excellent clinical skills
• Must comply with HIPAA rules and regulations
• State licensure and board certification is required
• Initiate and document appropriate interventions
• Evaluate and record patient and family response within practice protocol
• Prescribes drug therapies per protocol/physician consultation
• Assess patient and family needs and provides education
• Promotes/provides staff education
• Serve as a liaison between inpatient, skilled nursing, and home setting
• Incorporates evidence-based research into clinical practice
• RN State License and current registration with the State Board of Nursing in practicing state, along with current state certification of Nurse Practitioner

Master of Science in Nursing (MSN) from an accredited program for nursing education and successful completion of a Nurse Practitioner program

• Possess a minimum experience of six (6) months in the last twenty four (24) months as a Nurse Practitioner in a primary care outpatient setting (defined as family practice, urgent care, acute care).
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care.
• Participate in multidisciplinary team meetings to discuss patient cases and treatment plans.
• · Act as support and resource for new and current providers while addressing clinical concerns.
• · Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic tests. Diagnose and treat acute and chronic health conditions.
• · Develop and implement patient-centered treatment plans, including prescribing medications, therapies, and lifestyle modification
• Review key performance indicators frequently to ensure we are meeting facility goals including reviewing trends and creating action plans to reduce readmissions to hospitals or polypharmacy, for example.
• Provide health education and counseling to patients and their families.
• Collaborate with providers on treatment plans for patients as needed and work to improve facility KPI’s as needed.
• Provide on call support for several divisions as needed.
• Maintain accurate and up to date patient records in electronic health record
systems.
• Document patient care, assessments, and treatment plans in accordance with regulatory standards and best practices.
• Ensure compliance with healthcare regulations, standards of care, and
professional ethics.
• Stay current with advancements in healthcare by attending conferences, workshops, and continuing education programs.
• Travel within the designated regions to provide healthcare support in various healthcare facilities.
• Engages with providers to improve retention and operational success.
• Responsible for maintaining professional relationships with all partners and client leadership teams.
• Keep and maintain appropriate records relating to all professional services rendered and shall attend to all billing reports, claims and correspondence required, including supporting notes, and charts in connection with services.

Education and Experience:

· Master of Science in Nursing (MSN) with a Nurse Practitioner or Physician
Assistant specialization.

· Demonstrated ability to lead, train, develop and coach other APP’s.
· Previous experience leading a team highly preferred.
· Excellent communication and interpersonal abilities.
· Proficiency in electronic health record systems.
· Ability to work independently and collaboratively in a multidisciplinary healthcare team
· Willingness to travel within the regions as needed.
· Full COVID vaccination or exemption.
· Experience with geriatric population/long term care/short term rehab desired.

Are you a skilled and compassionate Family Nurse Practitioner looking to make a positive impact on patient care? We are currently seeking an experienced Family Nurse Practitioner to join our team, providing direct primary care services in our clinic and through telemedicine, as well as delivering mobile urgent care services.

1. Direct Primary Care Services:
• Conduct comprehensive medical assessments, diagnose illnesses, and
develop individualized treatment plans for patients ages 3 to 65 years
• Manage low complexity chronic conditions, provide preventive care, and
promote overall wellness through patient education and counseling.
• Collaborate with other healthcare professionals to ensure coordinated and
holistic care for patients.
• Utilize telemedicine technology to deliver virtual consultations and follow-up
appointments with patients as necessary.
Work four 9-hour days * 20-25 patients per day; carry your own panel of patients *
Adults and geriatrics, with some pediatrics * No call expectations at this time * Join
2.Physicians in a well-established practice * Manage patients with acute/chronic/complex conditions The Nurse Practitioner (NP) is responsible for the provision of health care services to individuals, families, and/or groups who receive their care under the auspices of medical group. Health care services include conducting health assessments, prescribing/providing
treatments/interventions, promoting health, and providing disease prevention and management. Health care services are provided under the direction and supervision of assigned primary care physician (collaborative agreement). The NP is expected to participate in quality assurance and improvement activities, engage in member of the team activities, and use resources (human and material) appropriately. Nurse Practitioner experience preferred. The NP maintains
continuing education credits of as prescribed by licensing body and the medical group and maintains prescriptive authority; acquires special clinical competencies as required by the group; participates in quality assurance and utilization
management activities; and assists with risk management functions.

Qualifications:

• 1+ years of primary care experience required
• Must love building relationships with patients, value bedside manner, and possess excellent clinical skills
